Effective immediately, a credit card will be required to book an appointment online. Patients who provide less than 24 hours notice, or miss their appointment, will be charged the full cost of the appointment to the credit card on file. Thank you for your understanding.

Please self-screen for any symptoms/illness prior to any appointment. Patients that display any symptoms in office will be required to wear a face mask. If feeling unwell, please notify the clinic in advance and the appointment will be rescheduled.
